PARENT’S CONSENT/PERMISSION

I hereby acknowledge and permit that my child/children to participate in the full range of program activities except as specifically noted by me in the health information section of the registration (where applicable).

 

I hereby agree that in the event of an accident, injury or illness involving my child/children, that I cannot be reached at the time or if the emergency is such that time does not permit such contact, SPLC officers, directors and employees is authorized to act on my behalf in case of an emergency and to take appropriate steps to have a doctor attend to my child/children, with no liability on the part of SPLC and their employees. I hold SPLC, their officers, directors and employees harmless from any and all actions, claims, damages and liabilities of any nature, arising from any decisions by SPLC or injury to my child/children that are not the result of negligence on the part of, or are entirely beyond the reasonable control of SPLC and its officers, directors and employees.

 

PHOTO AND VIDEO CONSENT

I hereby agree to allow photographs and/or video recordings to be taken of my child/children, to be reproduced, published, displayed, broadcast, including without limitation on SPLC internet web sites, in SPLC printed materials, or in any other materials or medium, based activities or events that include real people, for marketing, advertising, promotional and/or communication purpose. 

 

SICK POLICY

For the protection for all of the child/children in SPLC as well as the health of my child/children, in the event that my child/children becomes sick, the parents/guardians or emergency contacts will be notified and prompt pick up arrangements will be made. If my child/children is unable to play outdoors, and/or unable to participate in regular daily activities, alternate arrangements for care must be made. Child/Children with infectious illness, flu, fever, diarrhea, vomit, unexplained rash, conjunctivitis/pink eye CANNOT attend.

 

FIELD TRIP PERMISSION

I hereby give permission for my child/children to participate in each and every field trip and outing, with 2022 SPLC Summer Camp. I also give permission for my child/children to travel by school bus to any field trip that transportation is necessary. I understand that we will be advised of the field trips one-week prior. I declare that my child/children is physically fit and ready to participate in the field trips. I will not hold liable SPLC for any accident or injury occurring on or arising from the normal course of any field trip. I do authorize emergency first aid care to my child in the event that my child/children becomes injured or ill during the field trips.

 

ASSUMPTION OF RISK AND WAIVER

The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization and COVID-19 is extremely contagious. SPLC has put in place preventative measures to reduce the spread of COVID-19; however, SPLC cannot guarantee that the Participant will not become exposed and/or infected with COVID-19, and such exposure/infection may result in personal injury, illness, permanent disability, or loss of life.

 

By submitting and signing this form, I, a parent or guardian of the child/children acknowledge that there is potential risk for injury and/or contacting COVID-19 or any other contagious disease involved in activities. I understand that SPLC have tried to create a safe and controlled environment for participation and that it has established rules for participation on and that must be followed by the participant. I understand that failure to comply with any of the policies and rules of SPLC may result in the suspension or termination of camp. I waive the rights of the participant to damages or other costs in the event injury is caused due to participation in activities in SPLC. I understand that it is my responsibility to ensure that the information on this form is kept current and I will notify SPLC of any changes immediately.

 

Screening:

Parents/guardians are encouraged to check their child/children's temperature daily at home, however, a fever is not the only symptom of COVID-19

Parents/guardians and their child/children should line up along the front windows of the building and maintain a 2-meter distance from others when waiting to be screened.

 

Parents/guardians will sign off indicating that:

Neither their child nor anyone in their household has experienced any symptoms of COVID-19 in the last 14 days (symptoms may include: new or worsening cough, shortness of breath, sore throat, difficulty swallowing, new taste or smell disorder, nausea/vomiting, diarrhea, abdominal pain, runny nose or nasal congestion)

Neither their child nor anyone in their household have come into contact with anyone who has been a confirmed case of COVID-19

Neither their child nor anyone in their household have travelled outside of Canada in the last 30 days

 

** Individuals who answer YES to any of the questions will not be permitted inside the facility and will be sent home to seek assessment and further testing **

 

Your child's temperature will be taken and recorded. Any child with fever of 37 degrees Celsius or greater will not be permitted to enter the facility

Your child will be asked to sanitize their hands before entering the facility 

Attendance will be recorded.

 

PAYMENT AND REFUNDS

Payments and Completed Registration Form are due at registration.

Camp payments can be FULL refund 30 days before camp start date minus $30 administration fee and 50% refund minus $30 administrative fee within 30 days. NO refunds of camp payments will be made 7 days before camp start date. In the event the province of Ontario/York Region Public Health announces a mandatory shut-down of day camps and in the case there is an outbreak that we need to close, a full refund will be provided (for days not used).

 

EARLY DROP OFF AND LATE PICKUP

Please ensure prompt drop-off and pick-up of your child/children, SPLC has a early and/or late penalty charge of FIVE dollars ($5) for every five minutes, or part thereof, payable to the supervising staff member.

 

ITEMS TO BRING TO CAMP

Weather appropriate 2 full sets of spare clothing, a pair of indoor shoes if needed (such as crocs or flip flops) and reusable water bottle, snacks and lunch, face coverings (cloth mask or disposable mask).
